	Description of program mission, activities and goals*

	Name, location and extension of the Sutherland Global Services employee who will sponsor event coordination*

	Full permission to use the organizations logo and statements or pictures outlining the direct results of the charitable contribution.*

	How will your organization promote Sutherland’s donation or contribution? i.e. Press release, listed as a sponsor in advertisements, links on the web site etc…*

	Submit a short essay about how you feel that your organizations mission ties into our mission statement.*
